Our Policy & Release of Liability
REFUND POLICY:
NO CASH REFUNDS. (This means no refunds on credit cards or checks. If a customer decides to withdraw from our services, a UAC credit will be issued to the family in question with NO expiration date).
MAKE UP CLASS POLICY:
A make up class will be scheduled only in case of illness or injury with written verification from a physician. NO MAKE UP CLASSES ON SATURDAYS. A credit can be issued ONLY if there is no other class available for make up (example: the class only runs in ONE day of the week, or if the participant is in our Summer clinics).
BIRTHDAY PARTY POLICY:
Birthday Party payment must be received IN FULL in order for your party to be booked and processed.
Customers may reserve a date and time online for 48 hours without payment. Failure to complete payment within the 48 hour time period will result in cancellation of your reservation for that date and time.
The calendar does reflect the most up to date availability. Please contact the office at (203) 319-0087 to confirm availability.
Please be sure of the date and time, no last minute changes or cancellations allowed.
If a customer decides to withdraw from having a Birthday Party two weeks before the date, a UAC CREDIT IN FULL will be issued to the family in question, with no expiration date.
*Sorry, no confetti or piñatas are allowed at our parties.
Snow cancellations will be rescheduled for the next available date and time.
FIELD RENTAL POLICY:
All team managers/person-in-charge must fill out a form containing contact information, such as: phone number (home and/or cell/work) and address.
All players are required to sign a Release of Liability form before going onto the field.
Payment for rental is required at the time of booking.
When paying by cash, always request a receipt (it should contain at least the date and time of rental).
Please make all checks payable to United Athletic Centers.
For long term rental:
United Athletic Centers requires the following for long term rentals:
A deposit is required at the time of booking the reservations.
The deposit will not be lost if cancelled at least two weeks prior to the cancellation date.
Any deliquency in payment will result in losing the day/time requested.
GENERAL RULES OF THE FACILITY
Individuals utilizing this facility do so at their own risk. The property owner(s), and staff of United Athletic Centers(UAC) assume no liability for any injuries or accidents that may occur. Please reference Liability/Injury Waiver, Release Forms and signage posted within the facility.
Conduct within the facility and on the field should be in the spirit of good sportsmanship. Please NO ALCOHOL OR DRUG USE prior to playing.
Should you find or lose any items, please report this immediately to the UIS staff.
NO ALCOHOL, SMOKING, GLASS CONTAINERS, OR GUM ALLOWED IN THE FACILITY.
NO FOOD, DRINKS, SPITTING, OR ABUSIVE LANGUAGE ALLOWED ON THE FIELD.
We reserve the right to refuse play or service to anyone.
